This form can only be reached by great anger. Also, you have to have a pure heart, evil or good.
Goku was the first to reach it in a thousand years.
His transformation was caused by Krillin's death.
When Freeza killed him Goku became so mad he transformed. After that, Freeza was no match for him
Future Trunks was the second to transform in the series. He transformed by Gohan's death. He too kicked Freeza's ass.
Vegeta was the third. Against the Androids he showed his power with great pride. He did it by just not caring at all. When he was training in space he came upon a planet where a storm was raging. At first he liked it but the storm and meteorites threatened his ship. He was smacked down by a meteorite and then, he didn't care anymore. That was the time he transformed.
Gohan transformed in the Room of Spirit and Time.
He was so mad about not being able to help his friends he triggered the transformation.
Goten and Chibi Trunks became it very easily. They hadn't gone mad with anger like the other Saiyans. They just transformed.
You also have Brolly, but he only appeared in the movies. He was very strong but defeated by Goku.

Trunks rached this state in th Room of Spirit and Time. He thought he was stronger as Cell and he was. In brute strength that is. In speed Trunks was outnumbered by Cell.
Vegeta knew the limitations of this form and did not use it.
Goku too knew the limitations and told Gohan he wasn't going to use this form. Gohan was very surprised by this decision.
Brolly had the strength of a SSJ 3 in this form and he wasn't that slow.

SSJ 3

Gohan was the first to reach it during the Cell-Games. He became agian very mad and released all his powers and transformed. Cell wanted to see his sleeping powers and ordered the Cell Jnr's to fight the Z-Fighters. But Gohan still wasn't ready for his powers. Androids 16 head told Gohan to not be afraid for his powers. Then Cell stamped on 16's head. That was it for Gohan.
Goku did this while training in the afterlife.
Vegeta 'cheated' for this form. He let Babidi take control over him so he could fight and destroy Goku. But Vegeta finally became truly good and self-destructed himself near Buu. Buu did survive.

SSJ4

Goku achieved this form in the afterlife and used it to fight Buu. Although he was far more powerful as Buu, Goku used up all his energy he got for one day so he had to return to the afterlife.
Gotenks achieved it in the fusion. Goten and Trunks alone can't transform into this form

SSJ 4 Gogeta
|
Goku achieved this form first. This what Vegeta calls the true form. Goku achieved it by seeing Pan cry when he was in Golden Oozaru.
Vegeta cheated for this form by using Bulma's Brute Ray. So Vegeta maybe right about the true form

SuperSaiya-jins
|
This is the most powerful being in all the series. Vegeta SSJ 4 and Goku SSJ4 fused into one being with Goku in control.
This form can not be beaten
